Create a Form Without a Published Board

You can create a form without using an already published board if you prefer to start with a blank canvas. This is the second method of creating a form (see Create a Form Workflow).

To create a form

1. In the Form Gallery, click + Create Form.

2. In the Create Form window, enter the following information.

   Create Form fields

   Field	Description	Procedure
   Name	Name of the form. The name of the form is also used for the header (title) of the form.	Between 2 - 50 alphanumeric characters, excluding special characters.
   Description	A description of the form's purpose or function.	Between 2 - 500 alphanumeric characters, excluding special characters.
   Friendly Name	A short name that is used in the URL to make it more user-friendly. For example, if the URL is: https://forms.dev.juvare.com/forms/00344ff0-4d53-403c-aa13-88c58873ca19 Adding a friendly name of Vehicle-Checkin, changes the URL to: https://forms.dev.juvare.com/[org_name]-Vehicle-Checkin	Between 2 - 500 alphanumeric characters, excluding special characters. No spaces.
   Drag and Drop Area	A drag and drop area that allows you to use an existing form.	.Json file.

   

3. Click Done.

   • The form is created and appears in the Form Gallery.

   • You enter Design mode for the form.

You can now design your form. Go to Add Form Components.
