Entry forms cannot be edited or deleted, but they can be viewed by navigating to the Entry Forms module, then clicking View next to any form in the list.
The entry form will display a list of fields in the Fields tab, and a list of symbols that are shown in Crisis Track maps for those forms in the Symbology tab.
